All new eligible employees must complete the Enrollment Forms and return it to HR within 30 days of their eligibility date.

If you want to purchase more life insurance, in addition to what CDI provides to you, you can enroll in Optional Life/AD&D coverage with Mutual of Omaha.  You will also need to complete a Statement of Health Form if your coverage election exceeds the guarantee issue amount – please read about the optional life/AD&D enrollment process in the Benefit Guide.
