What you need to do

All forms must be completed and returned to HR by Friday, November 17. Your FSA participation will end if you do not re-enroll. You can complete the fillable PDFs and email to HR, or complete the paper forms in the enrollment packet and return to HR.

The health plans are designed to offer you comprehensive coverage as well as to protect you from heavy financial burden in the event of a catastrophic illness or injury.

Enrollment Forms

All new eligible employees must complete the Enrollment Forms and return it to HR within 30 days of their eligibility date.
